【摘要】目的:讨论超声诊断老年冠心病颈动脉硬化的临床效果。方法:研究时间为2020年1月-2020年12月,以同期50例老年颈动脉硬化患者作为研究组观察对象,另取50例健康体检者作为对照组观察对象,对两组研究对象接受超声检查后的检查结果及血流动力学测定结果进行观察。结果:超声诊断下两组研究对象的斑块积分、斑块厚度、颈动脉内膜中层厚度及动脉内径指标存在明显的统计学差异,P<0.05。超声诊断下两组研究对象的各项血流动力学测定结果指标存在明显的统计学差异,P<0.05。结论:超声诊断老年冠心病颈动脉硬化病症有一定的临床价值,且有无创、操作简便等优势,值得推广使用。

冠心病的临床患病率较高,其中城市老年男性群体患有此病的几率相对较高,典型胸痛为此病的主要患病症状表现,但对于部分患者而言,该病症并不明显,仅会出现心悸、心前区不适以及乏力等症状。动脉硬化为诱发冠心病的关键性因素,该症状最容易出现于机体的颈动脉,因此在诊疗冠心病时,应对患者进行颈部血管检查,以超声诊断老年冠心病颈动脉硬化病症的敏感性较高,且操作简便、无创[1]。基于此,本次研究中对老年冠心病颈动脉硬化患者接受超声诊断疾病后的临床效果进行了研究观察,如下。
1.资料与方法
1.1一般资料
研究时间为2020年1月-2020年12月,以同期50例老年颈动脉硬化患者作为研究组观察对象,另取50例健康体检者作为对照组观察对象。组间基线资料存在可比性(P>0.05):对照组中男/女:27/23,年龄均值为(70.9±6.4)岁;研究组中男/女:29/21,年龄均值为(70.6±6.3)岁。
1.2方法
所有入选对象均接受超声检查。要求患者采取仰卧位体位姿势,将探头频率调整为5.0-8.0MHz,协助于其保持仰卧位体位姿势,自其颈总动脉处起至近心端进行连续扫描,应先横断,再纵断,细致化对其颈总动脉的窦部、近端、远端、外动脉及颈内动脉进行扫描,使之有效获得清晰的纵断图像及血管横断。对患者的内-中膜厚度、颈内动脉起始、颈动脉分叉部、双侧颈总动脉以及颈外动脉处进行扫描与测定。若已判定患者狭窄程度>50%时,即可认定其存在冠脉病变,以内-中膜厚度判定斑块,并结合斑块结构的差异性,分为不稳定型与稳定型两种类型。
1.3观察指标
对两组研究对象的超声检查结果及血流动力学测定结果进行观察。
1.4统计学分析
计量资料用t计算,若P<0.05,即可判定存在统计学差异。

2.1超声检查结果比较
超声诊断下两组研究对象的斑块积分、斑块厚度、颈动脉内膜中层厚度及动脉内径指标存在明显的统计学差异,P<0.05,见表1。

动脉硬化与机体全身动脉疾病发生代谢异常有很大的联系,特别对于冠心病伴颈硬化病变来说,多发生于冠状动脉、主动脉、脑动脉中,该病症将致使机体的管壁出现破裂出血现象以及管腔闭塞现象,临床上通常将颈动脉硬化斑块的形成作为判断冠心病患者病情的关键性指标[2]。由于诱发冠心病的重要性原因之一为颈动脉硬化,因此,我国卫生部门正在积极推广血管病变检测技术,而对机体颈动脉功能异常进行判断的重要性手段之一就是采取颈动脉超声检查。
超声检查是集合测量、成像、血流信息采集为一体的诊断方法,该项临床检查不仅可清晰的呈现扫描图像,且可为医师提供丰富的血流动力学信息,因此该检查方式在临床上受到了医患的广泛性认可。以超声方式诊断老年冠心病颈动脉病症时可利用高频探头检测出血管内<1mm的钙化点,因此,可实现对颈动脉硬化性闭塞病的有效诊断。超声微血管成像技术主要基于彩色多普勒超声发展而来,具有较高的分辨率及敏感性,且可更为清晰的捕捉到患者的血流情况,高清的图像成像技术可便于对患者硬化性斑块情况进行有效显示,另外,超声检查还具有无创、无辐射、操作简便、可重复性操作等优势[3]。本次研究中发现,超声诊断下两组研究对象的斑块积分、斑块厚度、颈动脉内膜中层厚度、动脉内径及各项血流动力学测定结果指标存在明显的统计学差异,P<0.05,说明冠心病的产生与机体颈动脉硬化有很大的联系,实施超声检查利于对患者病情风险程度的评估,可帮助医师对患者的治疗方案的制定及治疗效果的评估提供科学依据。发现颈动脉硬化病症后,应转变饮食、运动方式,并积极遵医嘱接受降糖、降脂及降压治疗,并可结合颈部斑块大小实施针对性手术治疗[4]。
总结:超声诊断老年冠心病颈动脉硬化病症有一定的临床价值,且有无创、操作简便等优势,值得推广使用。
