摘要:目的:分析慢性阻塞性肺疾病合并重症呼吸衰竭应用无创呼吸机进行治疗的护理措施。方法:选取我院在2020年1月至2020年12月收治的84例慢性阻塞性肺疾病合并重症呼吸衰竭患者作为研究对象,将其随机分为对照组和观察组,对照组42例,实施常规护理干预,观察组42例,实施综合护理干预,比较两组患者动脉气血变化情况和治疗效果。结果:与对照组相比,观察组动脉气血变化情况和治疗效果优于对照组(P<0.05)。结论:慢性阻塞性肺疾病合并重症呼吸衰竭应用无创呼吸机进行治疗可以显著提高治疗效果,改善患者动脉气血变化情况,可用于临床推广。

对慢性阻塞性肺疾病合并重症呼吸衰竭患者来说,正确的治疗方法十分重要,临床上常采用有创机械通气,但其创伤大,容易引发其他并发症,利用无创呼吸机治疗慢性阻塞性肺疾病合并重症呼吸衰竭创伤较小,应用便捷,患者易于接受,此研究选取了我院在2020年1月至2020年12月收治的84例慢性阻塞性肺疾病合并重症呼吸衰竭患者作为研究对象,报告如下。
1资料与方法
1.1一般资料
选取我院在2020年1月至2020年12月收治的84例慢性阻塞性肺疾病合并重症呼吸衰竭患者作为研究对象,将其随机分为对照组和观察组,对照组实施常规护理干预,观察组实施综合护理干预。
对照组42例,男性患者20例,女性患者22例;年龄52~85岁,平均年龄(62.48±7.21)岁;病史1~21年,平均(9.42±7.63)年。
观察组42例,男性患者21例,女性患者21例;年龄51~87岁,平均年龄(65.97±7.45)岁;病史1~20年,平均(9.12±7.64)年。
两组慢性阻塞性肺疾病合并重症呼吸衰竭患者一般资料无差异性,无统计学意义(P>0.05)。
1.2 护理方法
对照组实施常规护理干预,主要包括监测患者生命体征,清理患者口腔分泌物,保证患者室内空气流通。
观察组实施综合护理干预,主要内容包括:
(1)心理护理:慢性阻塞性肺疾病合并重症呼吸衰竭患者极易出现焦虑等不良情绪,护理人员要与患者进行沟通,向患者详细解说采用无创呼吸机的必要性,使患者配合医生进行治疗,减少消极情绪。
(2)体位护理:根据患者需求为患者选择舒服的体位,保证患者头颈肩不处于同一条水平线,已达到呼吸通常的目的。
(3)并发症护理:适时调整患者位置,间歇性摘除面罩,缓解患者症状,鼓励患者主动咳嗽,护理人员也可以帮助清理口腔。
1.3评价指标
通过观察HP(次/min)、R(次/min)、PaO2(mmHg)、PH值四种指标评估患者综合护理干预前后动脉气血变化情况。
治疗效果分为显效、有效、无效三种情况,显效:慢性阻塞性肺疾病合并重症呼吸衰竭症状基本消失;有效:慢性阻塞性肺疾病合并重症呼吸衰竭症状有所缓解;无效:慢性阻塞性肺疾病合并重症呼吸衰竭症状无变化。
1.4数据处理
本文所有慢性阻塞性肺疾病合并重症呼吸衰竭患者数据用SPSS24.0软件分析检验,独立样本t检验动脉气血变化情况计量资料,用(±S)形式表示,x2检验治疗效果计数资料,用[n/(%)]表述,当P<0.05时,说明存在统计学差异。
2 结果
2.1 动脉气血变化情况比较
与对照组相比,观察组动脉气血变化情况和治疗效果优于对照组(P<0.05)。
