摘要 目的:探讨非布司他降尿酸治疗对慢性肾脏病(CKD)伴高尿酸血症患者氧化应激反应的影响。方法:选取2018年5月-2019年12月本院收治的106例CKD伴高尿酸血症患者作为研究对象,按照随机数字表法分为对照组(n=53)和研究组(n=53)。其中对照组患者采用别嘌呤醇治疗,研究组患者采用非布司他治疗,两组患者均持续治疗12周。比较两组治疗前后血尿酸(UA)、血肌酐(Scr)、氧化应激指标[丙二醛(MDA)、超氧化物歧化酶( SOD) 、8-羟脱氧鸟苷(8-OHdG)]以及炎症因子[超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)]水平。结果: 治疗后,两组UA和Scr水平较治疗前均明显降低 ,且研究组明显低于对照组(P<0.05);两组MDA和8-OHdG水平较治疗均明显降低 ,SOD水平较治疗前明显升高,且研究组MDA和8-OHdG水平低于对照组,SOD水平高于对照组(P<0.05);两组hs-CRP和IL-6水平较治疗前均明显下降,且研究组的hs-CRP和IL-6水平均低于对照组(P<0.05)。结论:非布司他降尿酸治疗CKD伴高尿酸血症疗效确切,能有效降低血尿酸和血肌酐水平,明显抑制氧化应激反应,并降低炎症因子表达。

慢性肾脏病(CKD)是一种以肾脏功能或结构受损为主的疾病,发病率较高,患者主要表现为血尿、蛋白尿等[1]。CKD最为常见的并发症为高尿酸血症,是加重CKD的危险因素 之一[2]。CKD患者血清尿酸浓度异常上升,容易引起肾小管损伤,促进氧化应激反应及肾内炎症。临床上通常采用降尿酸对症(给予别嘌呤醇)治疗该类患者,能在一定程度上缓解患者的病情,但临床效果欠佳。近年来有研究发现非布司他对降低血尿酸作用更为显著,能有效延缓肾脏疾病进展[3]。但目前将其用于CKD伴高尿酸血症患者的相关研究尚少。本研究将探讨非布司他降尿酸治疗对CKD伴高尿酸血症患者氧化应激反应的影响情况,结果如下:
1 材料和方法
1.1一般材料
以随机数字表法将本院2018年5月-2019年12月收治的106例CKD伴高尿酸血症患者分为对照组(n=53)和研究组(n=53),本研究经医院伦理委员会批准。纳入标准:①临床确诊为CKD,且伴高尿酸血症;②无其他风湿类疾病者;③对本研究知情同意者。排除标准:①存在与精神相关疾病者;②近两个月服用过非布司他药物者;③合并重要脏器功能障碍者;④对治疗药物过敏者。其中研究组男24例,女29例;年龄20-65岁,平均(36.17±8.64)岁。对照组男31例,女22例;年龄22-68岁,平均(37.02±8.94)岁。两组一般资料间比较均没有明显差异(均P>0.05),有可比性。
1.2方法
对照组患者采用别嘌呤醇(广东彼迪药业有限公司,国药准字H44021368,规格:100mg/粒)治疗,口服,100mg/次,1次/d,连续用药24周。
研究组患者采用非布司他(杭州朱养心药业有限公司,国药准字H20130009,规格:40mg/粒)治疗,口服,40mg/次,1次/d,若治疗后尿酸指标小于360μmol/L,用药剂量减少至20mg/次, 连续用药12周。
1.3评价指标
分别于治疗前、治疗12周后,取5ml两组患者外周静脉血,离心后分离出血清。通过 全自动生化分析仪sunoab-1018(购自上海泰益医疗仪器设备有限公司)检测患者血尿酸(UA)和血肌酐(Scr)水平;通过硫代巴比妥酸反应法测出丙二醛(MDA)水平;通过黄嘌呤氧化酶比色法测出超氧化物歧化酶( SOD) 水平;通过酶联免疫吸附法测出8-羟脱氧鸟苷(8-OHdG)、超敏C反应蛋白(hs-CRP)以及白细胞介素-6(IL-6)水平。
1.4统计学方法
本研究数据以通过SPSS 22.0软件进行分析处理,当P<0.05则认为差异具有统计学意义。氧化应激反应指标等计 量 资 料以(±s)表示,采用t检验;计数资料以%表示,采用χ2检验。
2 结果
2.1比较两组治疗前后UA和Scr水平 治疗前,两组UA和Scr水平相比均无明显差异(P>0.05);治疗后,两组UA和Scr水平均明显降低 ,且研究组明显低于对照组(P<0.05)。见表1.

3 讨论
CKD最为常见的并发症是高尿酸血症,且会严重影响CKD的病情发展[4]。临床上通常给予患者口服别嘌呤醇进行对症治疗,能在一定程度上缓解患者的病情,但存在药物不耐受等局限性,患者临床疗效不够理想。有研究表明,氧化应激反应在CKD伴高尿酸血症发生发展中起到重要作用[5]。因此,抑制氧化应激反应成为治疗该类患者的关键点之一。
非布司他是一种黄嘌呤氧化酶抑制剂,可以通过选择性抑制黄嘌呤氧化酶从而抑制尿酸的合成,进而达到减少血清尿酸的作用,且非布司他合成药物-酶复合物较为稳定,对其他酶类不会产生其他影响,大部分不经肾脏排泄,对机体无明显不良反应,安全性良好[6]。
高尿酸血症主要是由血液尿酸过高引起的一种机体状态,而Scr主要由肾小球滤过排出体外,测定其水平可以直观反应患者肾功能[7]。本研究结果发现,治疗后,两组UA和Scr水平较治疗前均明显降低 ,且研究组明显低于对照组。表明非布司他降尿酸治疗CKD伴高尿酸血症能进一步降低血尿酸和血肌酐水平,改善患者病情。非布司他可以抑制氧化型及还原型黄嘌呤氧化酶,且与其结合产生的复合物具有较高的稳定性,可以高效、持续抑制UA的合成,进而缓解患者病情,改善肾功能[8]。
相关研究发现,高尿酸血症会引起尿蛋白、肾损伤等,可能与激活氧化应激反应有关[9]。氧化应激指标中MDA是一种过氧化反应的最终产物,该指标浓度能反应人体氧化应激情况;SOD可以清除机体氧自由基,有利于机体抗衰老;8-OHdG是一种氧化性加合物,会引起DNA损伤。本研究中,治疗后,研究组MDA和8-OHdG水平低于对照组,SOD水平高于对照组。说明非布司他降尿酸治疗对CKD伴高尿酸血症患者能显著抑制氧化应激反应。CKD伴高尿酸血症患者由于细胞机制要平衡胞内浓度尿酸水平,会诱导氧化应激反应,进而激活炎症通路,促进合成促炎因子(hs-CRP、IL-6等)[10]。因此,患者血清炎症因子水平和氧化应激反应密切相关。本研究进一步对患者血清炎症因子水平进行分析,结果发现:治疗后,研究组hs-CRP和IL-6水平均明显低于对照组。可见非布司他能进一步降低炎症因子表达。这可能与非布司他能选择性抑制黄嘌呤氧化酶,可以高效、持续地抑制患者血尿酸的合成,改善内皮损伤程度,并显著抑制氧化应激状态等有关。
综上所述,非布司他降尿酸治疗CKD伴高尿酸血症疗效确切,能有效降低血尿酸和血肌酐水平,明显抑制氧化应激反应,并降低炎症因子表达。
