【摘 要】:目的 探究老年膝关节软骨损伤,以关节镜微骨折术治疗价值。方法 86例老年膝关节软骨损伤患者,随机分为A、B组,分别实施关节镜清理术、关节镜微骨折术,分析组间差异性。结果 术后1个月 B组CRP、IL-6、TNF-α水平较A组低,术后1个月HSS评分较A组高(P<0.05)。结论 对老年膝关节软骨损伤治疗中,应用关节镜微骨折术,可降低患者炎症因子水平,改善其膝关节功能。

老年钙合成能力下降,并随膝关节运动磨损,膝关节软骨易出现破碎、撕裂等现象,诱发膝关节炎症反应,引发炎性疼痛,影响关节功能[1]。手术为该病主要治疗方式。关节镜微骨折术,可促进患者骨生成,改善预后,但是否可增加患者创伤仍有待研究。为此,本次研究选86例老年膝关节软骨损伤患者,探究该术式临床价值。
1.资料与方法
1.1一般资料
86例老年膝关节软骨损伤,影像学确诊;患者均选于2019年1月至2020年6月;随机分为A、B组;排除凝血功能障碍、合并感染、合并恶性肿瘤、研究期间失访者;组间资料相近(P>0.05),研究经我院伦理委员会审核通过,见表1。

1.2方法
A组(关节镜清理术):硬膜外麻醉,仰卧位;关节镜下,以刨刀、探针、刮匙等工具清除膝关节损坏软组织病灶,去出病灶位置松动软骨,修整下骨及软骨缺损边缘,缝合撕裂软骨,完全清除后,关节腔以无菌生理盐水反复冲洗,直至碎屑完全清除,退出关节镜。
B组(关节镜微骨折术):硬膜外麻醉,仰卧位;切口位置选膝关节前内外侧做切口,置入关节镜后,观察病灶位置软骨状态,清除碎骨,软骨边缘垂直整理,使之形成池状;清理后目标关节面以微骨折尖钻孔处理,保证钻孔与骨面垂直,保证微骨折钻孔均匀分布,空隙之间骨桥3~4mm;刮匙清理微骨孔附近骨屑,吸尽关节中液体后退出关节镜。
两组患者术后均接受抗感染、冰袋冷敷、康复锻炼等干预。
1.4观察指标
(1)比较两组术前、术后1个月炎症因子,以酶联免疫吸附法检验C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤细胞坏死因子-α(TNF-α);(2)比较两组术前、术后1个月膝关节功能,以膝关节功能评分(HSS)[2]表示,满分100分,分数与膝关节功能呈正比。
1.5统计学方法
(%)表示患侧例数占比等计数资料。(±s)表示炎症因子水平等计量资料,2、t检验;P<0.05提示差异显著。数据以SPSS24.0软件计算。
2.结果
2.1术前、术后1个月炎症因子
术前两组炎症因子水平相近(P>0.05);术后1个月炎症因子:B<A(P<0.05),见表2。

3.讨论
关节镜下清理术为老年膝关节软骨损伤常规术式,通过彻底清除病灶组织、清理软骨缺损边缘,以清除炎性介质,恢复患者术后短期膝关节功能,对彻底改善患者膝关节功能有限;并随术后关节使用而增加其软骨损伤,诱发机械磨损型血清炎症反应,复发疼痛、活动受限等状态,影响患者膝关节远期康复能力,因此治疗效果有限[3]。
关节镜微骨折术,在传统关节清理术基础上,通过钻孔修复,使骨髓细胞渗入至微钻孔内,使血块凝结填充软骨缺损,使之在软骨修复调控机制下形成纤维软骨,促进患者损伤软骨修复,以恢复患者膝关节功能[4]。本次研究结果发现,B组术后1个月炎症水平较A组低,HSS评分较A组高,考虑原因为,与传统清理术相比,关节镜微骨折术可在不大量产生热量基础上,增强骨髓刺激,并利用关节镜技术可多角度行骨面垂直操作,避免破坏膝关节力学结构;并通过骨髓细胞渗透补充缺损软骨结构,改善患者膝关节活动时疼痛、肿胀、活动受限等症状;同时随其膝关节软骨结构改善,可减少软骨退行性病变损伤引发的炎症反应,达到降低患者血清炎症反应指标、改善膝关节功能之效[5]。
综上,对老年膝关节软骨损伤治疗中,应用关节镜微骨折术,可降低患者炎症因子水平,改善其膝关节功能。
