【摘要】目的:观察并评价慢性心衰采取强心通脉颗粒治疗的效果。方法:将70名患有慢性心衰的患者作为观察对象,随机分成两组:一组使用常规抗心衰治疗策略(对照组)、另一组再予以使用强心通脉颗粒治疗(观察组)。通过观察治疗前、后的有关情况评价组间疗效差异。结果:治疗前,两组患者心脏彩色多普勒超声心动图测定LVEF、CO、EDV、ESV值差异并不大(P>0.05);治疗后,与对照组比较,观察组患者LVEF、CO值更高,EDV、ESV更低(P<0.05)。结论:强心通脉颗粒用于慢性心衰治疗,可明显提高患者左室射血分数、心输出量,改善心功能。

慢性心衰是由多种原因导致心脏结构或功能受损而发生的疾病,多见于老年群体中,是多种器质性心脏病的后期表现,因其病症危重,所以也是器质性心脏病的主要致死原因。强心通脉颗粒既能益气、又能活血,对慢性心衰患者的疾病治疗有显著疗效。下文研究在常规治疗的基础上提出加用强心通脉颗粒治疗,见具体总结:
1病例资料和方法
1.1基础资料
2019年8月~2020年8月在我科住院患者,结合其病史、体征、心电图等明确慢性心力衰竭,心功能Ⅱ~Ⅲ级患者总计70例。将有以下情况者排除:(1)近半月有相关中药服用史;(2)急性心功能不全、致命性心律失常和其它心脏疾病患病者;(3)心衰合并未被控制的感染、严重脏器功能不全、严重电解质紊乱者;(4)妊娠、哺乳期、过敏等特定情况者。随机分两组后:对照组35例,男20例、女15例,年龄21~72岁,平均46.3±3.5岁,冠心病16例、高血压病17例、扩张型心肌病3例;观察组35例,男23例、女12例,年龄23~73岁,平均47.8±3.8岁,冠心病15例、高血压病15例、扩张型心肌病5例;两组基础资料存在可比性(P>0.05)。
1.2方法
对照组患者按照慢性心衰治疗指南采用常规抗心衰治疗方法,选用卡托普利片(初始1次口服12.5mg,每天2~3次,必要时可增量到50mg,每天2~3次)或马来酸依那普利片(初始剂量2.5mg/次,结合患者耐受度适量增加到常用2mg维持量,1次或2次分服),无法耐受上述药物者改用比索洛尔(初时使用剂量为5mg,每天1次,或可相应调整剂量到10~20mg)、地高辛片(常用0.5~2片,每天1次;若达到快速负荷量,每6~8h给药1片,总量每日3~5片;维持量每次0.5~2片,每天1次)、利尿剂等常规药物。
观察组另外使用强心通脉颗粒治疗,用量为每次1袋(15g)开水冲服,每天3次。
治疗疗程总计3个月。
1.3观察指标
在治疗前后,分别使用心脏彩色多普勒超声心动图测定两组患者左室射血分数(LVEF)、舒张末期容积(EDV)、收缩末期容积(ESV)和心输出量(CO)。
1.4统计学
使用SPSS25.0统计软件对上述观察指标做数据的检验,数据表达为均数±标准差的形式,使用t检验,检验结果P<0.05代表数据差异有统计学意义。
2结果
治疗前,两组患者心脏彩色多普勒超声心动图测定LVEF、CO、EDV、ESV值差异并不大(P>0.05);治疗后,与对照组比较,观察组患者LVEF、CO值更高,EDV、ESV更低(P<0.05)。表1:

中医认为慢性心衰的病因为心病久延,造成气血的阴阳损伤,随着各个脏腑功能的失调,使得血脉通行不得顺畅,水饮内停,继而导致淤血停滞,最终发病。中医提出:心主血脉,心脏是推动血液正常运行之根本,同时也有赖于心气的正常作用,如果心气欠弱,邪气扰心,那么在血运无力的状态下,心悸、气短等心衰症状便会发生,可以说,心气虚是慢性心衰疾病发生之根本。心气虚而致血运无力,血气虚而致血脉瘀阻,血气虚而致血液运行失畅、水液代谢异常。心衰病位主要在心,和脾、肺、肾有着密切关联,属本虚标实之证,气虚为本虚,血瘀为标实[1]。
故而,在本文研究中提出采用强心通脉颗粒来治疗慢性心衰,其药物的组成有人参、黄芪、丹参、红花、三七、益母草、附子、桂枝等,其中人参和黄芪是为君药,可振奋心阳,促进气血顺畅运行,消除瘀血;丹参、红花和三七则均能够活血化瘀;益母草除了能够活血、祛瘀之外还能利水,入心包经,既能引经、又能消水;附子有着散寒、止痛的功效,桂枝则有助阳化气、平冲降气的作用,全部方剂的协调使用,皆能起到益气、活血之效,以补其虚、祛其瘀而实现治疗心衰的作用[2-3]。
由上文研究数据结果证实,观察组在常规抗心衰治疗的基础上采用强心通脉颗粒治疗后,患者左心室射血分数与心搏量皆更见提高。是以既突出了西医常规药物的治疗特点,而且又见中药的标本兼治,以多途径来实现慢性心衰的治疗优势。
综而以上全文,强心通脉颗粒用于慢性心衰治疗,可明显提高患者左室射血分数、心输出量,改善心功能。
