【摘要】 目的 探究酒精性肝病合并乙型肝炎病毒感染的临床特点。 方法 实验开展时间为2018年5月-2020年5月,选取70例酒精性肝病合并乙型肝炎病毒感染患者为实验组,70例单纯酒精性肝病患者为对照组,对两组的治疗效果进行对比研究。结果 治疗前,实验组患者的各项肝功能指标包括AST、ALT、GGT、TBIL均显著高于对照组,治疗后,两组患者肝功能指标均有所下降,但实验组下降值均显著低于对照组,实验组患者乏力、肝性脑病、消化道出血等症状的发生概率显著高于对照组差异较为明显,P<0.05,存在统计学意义。结论 酒精性肝病合并乙型肝炎病毒容易加重患者的病情,造成肝功能的下降,增加并发症的发生概率,对治疗效果有着严重的负面影响。

酒精性肝病是一种常见的临床肝脏病症,该种病症是由于患者在日常生活中过量饮酒导致肝脏功能受损而引起的疾病。在临床研究中发现,肝病的主要原因通常是患者受到酒精或者肝炎病毒等外界因素的影响。乙型肝炎病毒是一种常见的病毒类型,会引起患者多个脏器功能的损伤,甚至出现肝癌或者肝硬化等病症。所以深入研究酒精性肝病合并乙型肝炎病毒感染的临床特点具有非常重要的现实意义[1]。本文以此为基础进行研究,结果如下。
1 资料与方法
1.1一般资料
纳入实验研究的70例试验组患者均经临床诊断确诊为酒精性肝病合并乙型肝炎病毒感染患者,70例对照组患者均经临床诊断为酒精性肝病,实验开展时间为2018年5月-2020年5月,实验组中,男女比例为38:32,最大与最小年龄分别为68岁和20岁,均值(48.96±4.72)岁;对照组中,男女比例为36:34,最大与最小年龄分别为67岁和20岁,均值(48.72±4.59)岁。一般资料数据差异不大,P>0.05。
1.2方法
两组患者在入院后,均指导患者进行烟酒的戒除,对患者生活和饮食进行指导。针对肝酶较高的患者,采用2.4克还原性谷胱甘肽注射液混合60毫升复方甘草酸苷注射剂通过静脉进行滴注,用量为每日一次;针对胆红素较高的患者,将1.5克丁二磺酸腺苷蛋氨酸注射液混入到250毫升浓度为5%的葡萄糖溶液当只能够,通过静脉进行滴注,用量为每日一次;实验组内有抗病毒指症患者,给予10毫克阿德福韦酯和100毫克拉米夫定通过温水冲服,用量为每日一次。对两组患者采取营养和病症的对症治疗。
1.3观察指标
对两组患者的肝功能指标包括谷草转氨酶(AST)、谷丙转氨酶(ALT)、谷酰转肽酶(GGT)、总胆红素(TBIL)进行记录和比较。
对两组患者临床症状包括乏力、消化道出血、肝性脑病发生情况进行记录和比较。
1.4统计学处理
将两组效果录入SPSS22.0软件,计数资料检验行x2,表示用(%),计量资料检验行t,表示用(±s),P<0.05是统计学意义成立的依据。
2结果
2.1两组患者治疗前肝功能指标和治疗后下降值进行比较
治疗前,实验组患者的各项肝功能指标包括AST、ALT、GGT、TBIL均显著高于对照组,差异较为明显,P<0.05,存在统计学意义;治疗后,两组患者肝功能指标均有所下降,但实验组下降值均显著低于对照组,差异较为明显,P<0.05,存在统计学意义。

根据资料显示,酒精性肝病和乙肝病毒感染两者会相互影响,发生叠加致病作用。乙醇中含有可以激活肿瘤坏死因子,会增加乙肝病毒X蛋白,对自然杀死细胞的生成有着抑制作用,会激活免疫系统中TLR4HE和补体,加快HBV基因的表达和复制,对肝炎的产生和发展起到促进作用,造成肝脏功能的损伤,对治疗效果有着严重的负面影响[2]。
研究表明,治疗前,实验组患者的各项肝功能指标包括AST、ALT、GGT、TBIL均显著高于对照组,治疗后,两组患者肝功能指标均有所下降,但实验组下降值均显著低于对照组,实验组患者乏力(21.42%)、肝性脑病(27.144%)、消化道出血(24.29%)等症状的发生概率显著高于对照组差异较为明显,P<0.05,存在统计学意义
综上所述,酒精性肝病合并乙型肝炎病毒容易加重患者的病情,造成肝功能的下降,增加并发症的发生概率,对治疗效果有着严重的负面影响。所以,针对酒精性肝病合并乙型肝炎病毒感染患者,要引导患者积极戒酒,减少引起疾病的因素,对疾病的发生和发展进行控制。
