摘要:目的:对高血压性脑出血病人的临床医治方法进行探讨,以实现提高急诊内科的临床救治效果。方法:从 2017 年 6 月—2019 年 10 月进行分析调查,期间从该院急诊内科收治的高血压性脑出血病人中挑选出 70 名作为该次研究对象,将其分组形式进行探究,共分为两组,其中行常规医治的 35 名病人为参照组,在参照组基础上实施院前急救医治的 35 名病人为实验组,对比两组病人最终医治效果。结果:参照组神经缺损功能评分(16.7±1.9)分明显高于实验组(12.3±2.6)分,差异有统计学意义(t=8.083,P<0.05),参照组抢救时间、医治时间明显长于实验组,差异有统计学意义(P<0.05),参照组病死率(25.71%)明显高于实验组(5.71%),差异有统计学意义(x2=5.285,P<0.05);实验组最终医治效果(97.14%)明显高于参照组(80.00%),差异有统计学意义(x2=5.081 ,P=0.024)。结论:对急诊内科高血压性脑出血病人而言,实施急诊医治,可缩短抢救时间和医治时间,改善临床症状,在临床医治中取得显著的临床疗效。

1 资料与方法
1.1 一般资料
该院对高血压性脑出血以分组方法进行分析,一组为实验组,一组为参照组,两组名数均等,均为 35 名病人。参照组,最大年龄为 84 岁,最小年龄为 62 岁,平均年龄为(71.4±1.6)岁;最大体重为 82 kg,最小体重为 45 kg,平均体重为(65.4±5.2)kg;其中包括 20 名心脏病病人,10 名高血压病史病人,5 名心脏病史病人;男性和女性分别为 20名和 15 名。实验组,最大年龄为 86 岁,最小年龄为 60 岁,平均年龄为(71.6±1.7)岁;最大体重为 85 kg,最小体重为43 kg,平均体重为(66.7±4.9)kg;其中包括 19 名心脏病病人,9 名高血压病史病人,7 名心脏病史病人;男性和女性分别有 19 名和 16 名。该次研究均在伦理委员会批准的前提下进行,所有病人家属均在了解该次研究的前提下签署知情同意书,所有病人均有高血压史,所有病人经 CT诊断均被确诊为高血压脑出血病人。两组病人年龄、性别比较,差异无统计学意义(P>0.05)。具有可比性。
1.2 方法
参照组实施一般医治方法,根据病人具体情况对病人实施止血、降低颅内压等医治办法,并根据病人具体情况为其制定合理的手术医治方案,尽可能减少手术医治风险,确保病人的生命安全。实验组病人在此基础上实施急诊医治,主要包括以下几方面内容:①急诊救治。根据病人具体情况以及家属所描述情况,急诊救治人员在病人赶往医院途中做好相关科室抢救准备工作。②气道管理,保证头偏侧位,以防呕物吐误吸,给与吸氧,必要时口咽通气管甚至气管插管。保证呼吸的基础上,若患者有烦躁不安,可给予适当镇静。③确认病人临床症状。若病人出现脑出血症状,应及时采取脱水医治方法,给病人静脉处注射甘露醇(国药准字 H11020861),甘露醇浓度为 20%,剂量为 250 mL;若病人病情加重,使用甘露醇无法缓解,则需给病人静脉注射呋塞米(国药准字 H31021063),注射剂量为 20 mg,注射间隔时间为数分钟 1 次。④联合降压医治。如果病人的血压太高,一种降压药不能起到降压的作用,就可以通过联合用药,普遍都是选择两种以上药物、小剂量的进行血压控制医治,可以通过钙离子拮抗剂或者β受体阻滞剂、利尿剂、转换酶抑制剂等药物的使用,在结合测量的血压结果进行联合用药,由于该用药方法不但可以起到很好的疗效,同时还能够降低出现不良反应的几率。
1.3 观察指标
①观察两组病人神经功能缺损评分。 主要利用美国立卫生院卒中量表进行评估,评估最高分为 42 分,最低分值为 0 分,临界值为 20 分,分值与病人病情成正比。②分析两组抢救时间和医治时间,时间越短,证明医治效果越好。 ③计算两组病人病死率。主要通过观察两组病人死亡名数,对病人病死率进行判断。④痊愈,病人再无临床症状,且神经功能大幅度改善,改善幅度<90%;显著进步,若病人临床体征和临床症状得到改善,且改善幅度>45%,<90%;进步,病人临床症状和体征没有发生变化,神经功能改善幅度>45%,≤90%;无变化,病人神经功能缺损情况未发生改变,且神经功能缺损改善幅度<17%。
1.4 统计方法
该次急诊医治临床效果主要使用专业的 SPSS 19.0统计学程序进行计算和统计,计量资料采用(±s)表示,行t 检验,计数资料采用[n(%)]表示,行 χ2检验,P<0.05 为差异有统计学意义。
2 结果
2.1 对比实验组与参照组神经功能缺损评分 、抢救时间、治疗时间
实验组神经功能缺损评分、抢救时间、医治时间分别为(12.3±2.6)分、(32.7±6.7)min、(26.4±1.6)min,均优于参照组,两组比较差异有统计学意义(P<0.05),见表 1。

2.2 对比实验组与参照组病死率
实验组病死率相较于参照组明显较低,差异有统计学意义(P<0.05)。见表 2。

伴随人们生活水平、生活方法的改变,越来越多的人有饮食不节、懒于运动、生活作息不规律等恶习,此种习惯导致人们的身体素质较差,高血压和糖尿病等疾病呈逐年上升趋势,尤其是高血压性脑出血疾病,发病概率越来越高,此类疾病会对病人的身体健康造成严重威胁。高血压性脑出血主要由病人颅内血压突然增高诱发局灶性缺血或缺氧形成的微小动脉瘤。当病人情绪出现较大波动或者不小心摔倒后会因血压突然升高导致脑血管破裂出血,此种病症具有起病急、发病突然、病情发展变化快等特点,需及时采取有效办法控制病情,改善病人临床症状,减少病变的可能,以免病人在短时间内死亡。
常规医治是一种传统医治方法,通常在病人达到医院后,经过一系列相关检查,明确病人病情后再实施医治办法,此种医治方法虽可能实现医治的目的,但是中途浪费了大量时间,使得病人病情无法得到及时控制,从而增加了病人死亡的风险;且此种医治办法即使控制住病情,预后效果也相对较差,病人身体会因疾病受到严重损伤,从而缩短病人生命周期,降低病人生活质量。
急诊医治主要通过在病人前往医院途中,医护人员便根据家属描述情况做好急诊救治工作,待病人入院后立即实施救治,稳定病人病情,尽可能减少疾病对病人神经功能的破坏,减少病人机体损伤程度,从而改善病人临床症状,保证病人的生命安全,降低病人的病死率。其次此种方法通过帮助病人调节情绪,消除病人负面情绪,减少病人紧张、焦虑感,让病人尽量保持放松状态,减少情绪波动,从而缓解病痛,有利于预后效果的提高。
该次研究表明,病人神经功能缺损评分(12.3±2.6)分vs (16.7±1.9)分 、抢救时间(32.7±6.7)min vs (47.6±7.2)min、医治时间(26.4±1.6)min vs(34.2±0.7)min、病死率 5.71% vs25.71%,实验组明显优于参照组 (P<0.05),观察组最终医治效果 97.14%明显高于参照组 80.00%(P<0.05)。由此可见,对急诊科高血压性脑出血病人而言,实施急诊医治明显优于常规医治方法,可最大限度地缩短病人医治时间和抢救时间,及早控制病情,减轻病人病痛折磨,提高预后效果,对于病人发病早期医治的意义重大,可为病人赢得最佳医治时间,延长病人生命周期,提高病人生活质量,促进病人身体的快速康复。相关学者研究 2 组抢救率成功率(86.36%)明显高于甲组(66.18%)(P<0.05)。此研究与该次研究结果一致。
4 结语
综上所述, 我们可以看出科学的急诊内科医治可作为后续医治的基础,可促进高血压脑出血病人的身体康复,降低病人的病死率和致残率,愈后效果较好。
