【摘 要】目的:分析托拉塞米(缩略词TOR)联合左卡尼汀(缩略词LC)治疗慢性心力衰竭(缩略词CHF)急性加重期的疗效。方法:研究时间介于2019年12月-2020年12月间,共计CHF急性加重期患者78例。以随机法为准,A组39例采取TOR联合LC治疗。B组39例采取LC单纯治疗。对比治疗效果。结果:A组的总有效率为97.44%,B组为82.05%;治疗后,A组的心室肥厚相关指标低于B组,对比数据有差异(P<0.05)。结论:为CHF急性加重期患者采取TOR联合LC治疗能够提升疗效,缓解心室肥厚表现,疗效较为理想。

CHF的发病过程为静脉回流充分,但心排血量大幅减少,导致组织器官代谢异常,病理特征为体循环与肺循环多程度淤血和血流灌注减少等[1]。其致病因素为高血压等基础病,且在心律失常等因素作用下,症状容易急性加重。CHF急性加重期的病情危险度高,治疗预后不佳,死亡风险极高。为此,临床建议采取药物治疗,常用药有TOR与LC等。本研究选取78例CHF急性加重期患者,用于分析TOR联合LC的治疗效果。
1资料与方法
1.1一般资料
研究落实于2019年12月-2020年12月间,统计78例CHF急性加重期患者。根据随机法分组,A组39例,男患比女患为21比18;年龄从37岁间断至76岁,均值(50.21±1.75)岁。B组39例,男患比女患为22比17;年龄从34岁间断至79岁,均值(50.58±1.67)岁。数据经假设检验并无差异(P>0.05)。
1.2 方法
B组使用LC单纯治疗,将2g LC(国药准字H20070286,海南通用康力制药)混入葡萄糖注射液(5%)中,制备成溶液100ml,每日滴注2次。A组基于B组,加用TOR治疗,TOR(国药准字H20040076,湖北百科亨迪药业)的口服剂量为每次20mg,每日1次。两组的疗程均为1周。
1.3 观察指标
观察治疗前后的左室舒张末径(缩略词LVEDd)、左心室舒张末期室间隔厚度(缩略词IVST)、左心室后壁厚度(缩略词LVPWT)等心室肥厚指标。
1.4 疗效评价标准
显著疗效:症状明显改善,心功能恢复至1级;初见疗效:症状有改善,心功能降低1个等级;未见疗效:症状无变化,心功能无改善[2]。
1.5 统计学分析
数据处理经由SPSS21.0软件完成,计量数据经t值对比与检验,计数数据经x2值对比与检验,假设校验有意义的标准为P值不足0.05。
2结果
2.1 对比两组患者的总有效率
A组的总有效率计为97.44%,B组计为82.05%,对比后差异显著(P<0.05)。

2.2 对比两组患者的心室肥厚指标
治疗前,治疗的心室肥厚指标对比后无差异(P>0.05)。治疗后,A组的心室肥厚指标低于B组(P<0.05)。

CHF的病理基础是心肌功能与结构发生变化,预后不良。CHF急性加重期典型症状是腹胀、咳嗽与呼吸困难等,主要体征为肺底湿罗音和肺部浮肿等,患者需要接受吸氧、体位调整和利尿剂等对症治疗。但以上疗法长期治疗存在较多的副作用,整体疗效不佳[3]。LC治疗后可保证脂肪酸代谢正常,其可以通过对外源性左卡尼汀的有效补充,使心肌细胞内部的脂酰辅酶A可以大量进入线粒体,从而减轻细胞多种代谢酶的实际活性,防止细胞膜破坏。其单纯治疗的利尿效果欠佳,而TOR是常用的袢利尿剂,其利尿速度较快,可以减少心脏前负荷量,增加体内的前列环素等物质含量,对于血栓素A2有拮抗作用,能够扩张血管,阻断醛固酮与受体的有效结合过程。其生物利用度高,具有极强的利尿效果,药效持续时间长,药物对于钾离子的排泄作用相对减弱,不影响机体电解质平衡,用药安全性高[4]。此外,TOR能够恢复患者的血流动力学,改善症状。同时能够抑制肾小管重吸收过程,使水钠尽快排出体外,进而优化心功能。以上两种药物联合治疗能够起到协同作用,尽快消除患者症状,具有增效作用[5]。
结果中,A组的总有效率高于B组,心室肥厚指标低于B组(P<0.05)。说明TOR联合LC治疗能够尽快改善CHF急性加重期症状,防止心室肥厚等表现,具有较高的临床治疗价值,可以作为该病的首选联合用药方案。
