摘要:目的:研讨信必可都保、舒利迭治疗缓解期慢阻肺的临床价值。方法:遵从“平衡序贯法”,将我院2019年2月到2020年4月内?66例缓解期慢阻肺分为2组,33例信必可都保治疗为对照组,33例舒利迭治疗为观察组。对比分析治疗安全性和患者肺功能。结果:治疗后观察组肺功能各指标水平高,P<0.05。观察组不良反应发生率为6.0%,对照组高达24.2%,P<0.05。结论:对缓解期慢阻肺患者实施药物治疗,临床更建议推广舒利迭,可有效改善患者肺功能并确保治疗安全性。

慢阻肺疾病以“长期气流受限”为特征,疾病进一步加重会损伤患者呼吸系统,进而严重影响患者日常生活。临床研究指出,缓解期慢阻肺患者的治疗应遵循“缓解症状、改善肺功能”原则,同时也是提高治疗效果、提升患者生命质量的根本[1-2]。为研讨信必可都保、舒利迭治疗缓解期慢阻肺的临床价值,特展开此研究。
1、资料与方法
1.1 一般资料
遵从“平衡序贯法”,将我院2019年2月到2020年4月内?66例缓解期慢阻肺分为对照组(n=33):男、女为16、17例,年龄55-83(69.66±11.52)岁;病程1-11(6.05±3.15)年。观察组(n=33):男、女为18、15例,年龄55-84(69.50±11.48)岁;病程1-10(5.69±3.56)年。借助SPSS 22.0软件分析两组资料,P>0.05。
1.2方法
此研究所用药物具体信息:信必可都保:瑞典:AstraZeneca AB,注册证号 H20140459。舒利迭:法国Glaxo Operations UK Limited,注册证号 H20150323。
对照组:信必可都保:严格以检查结果为准,指导患者戒烟并给予营养支持和低流量氧疗,经口吸入信必可都保,每次1吸,每天2次。
观察组:舒利迭:指导患者经口吸入舒利迭,每次1揿,每天2次。两组患者坚持治疗半年。
1.3观察指标
治疗前后肺功能情况:主要对比FVC、FEV1、FEV1/FVC;借助肺功能仪检测。
治疗安全性[3]:安全性主要从心悸、震颤、口干、咽部不适等不良反应发生率进行对比,不良反应发生率越低表示治疗安全性越高。
1.4统计学方法
建数据库借助SPSS 22.0进行统计,治疗安全性借助“x2”检验并作处理,肺功能设计“t”检验并处理,数据遵从正态分布原则,以“P<0.05”视为研究存在统计学显著。
2、结果
2.1 治疗前后肺功能情况
治疗后观察组FVC、FEV1、FEV1/FVC水平高,P<0.05。见表1。

慢阻肺是一种呼吸系统常见疾病,在多因素联合作用下,该病以中老年人为主要发病群体;随病情进一步发展,极易累及患者肺和气道等组织,同时还会诱发严重的全身反应[4]。目前临床根据慢阻肺患者症状又将其分为缓解期、急性加重期;而治疗该病仍以药物治疗方案最为常见,其中又以抗炎药、止咳药、祛痰药等药物最常使用。
信必可都保属于长效复合剂,内含福莫特罗、布地奈德,其中福莫特罗是 β2受体激动剂(长效型),迅速起效且作用持久,抗炎效果良好,药物可被支气管黏膜完全吸收,进而对炎性细胞的渗出进行抑制, 减轻气道炎症反应的同时持久舒张支气管。福莫特罗可增加细胞受体敏感性,增强抗炎活性。舒利迭内含沙美特罗、丙酸氟替卡松,其中沙美特罗脂溶性、选择性更高,所以药物作用时间更长;丙酸氟替卡松选择性较高且亲和力较强,因此药物抗炎作用强大,进而可对上皮细胞的增生进行有效抑制[5]。
此研究结果示:治疗后观察组肺功能各指标水平高,P<0.05。观察组不良反应发生率为6.0%,对照组高达24.2%,P<0.05。证实了舒利迭治疗缓解期慢阻肺的临床价值。提示:信必可都保与舒利迭相较,前者优越性在于起效短,但是其中的福莫特罗是完全的β2受体激动剂,所以药物作用与剂量之间呈正比关系,因此患者需要长期且大量的用药,但会导致后期不良反应发生率较高,所以治疗安全性较低;因此治疗缓解期慢阻肺疾病,临床更建议推广舒利迭。
