摘要:目的:探索改进院前急救护理流程在急性脑出血患者中的应用价值。方法:在急诊接收的患者中择取80例急性脑出血患者作为研究对象,摸球随机分组,两组中各有40例,对照组行常规院前急救护理,观察组行改进院前急救护理流程,分析两组的急救效果指标和并发症发生率。结果:观察组急救工作质效高于对照组且患者并发症发生率低于对照组(P<0.05)。结论:改进院前急救护理流程对提高急救工作质效并降低患者并发症发生率具有非常明显的作用,值得推广使用。

急性脑出血是一类临床高危疾病,该病发病迅速且具有极高的死亡率,患者需接受院前急救护理才能最大限度挽救其生命。随着现代医疗资源越来越紧张,常规院前急救护理措施已经不能满足当下护理需要,改进院前急救护理流程已迫在眉睫 [1-2]。为有效提高医院急救工作质效,本文将详细分析改进后的院前急救护理流程并与常规院前急救护理措施进行对比,从而体现改进院前急救护理流程的科学性和先进性,判断其临床应用价值。
1资料和方法
1.1基本资料
病例挑选年段: 2018年1月-2019年1月。
病例一般资料:急性脑出血患者共80例,摸球法随机分组。对照组:奇数号,共40例,男21例,女19例;年龄65-75(70.18±3.22)岁。观察组:偶数号,共40例,男20例,女20例;年龄63-75(70.16±3.19)岁。组间资料对比,P>0.05。
筛选标准:(1)符合2011年脑出血指南修订版标准的患者。(2)患者家属对本次试验知情并自愿代患者签署知情同意书。
排除标准:(1)发病后收缩压≥200mmHg,舒张压≥120mmHg的患者。(2)广泛蛛网膜下腔出血患者。(3)肝肾功能不全以及患有其他恶性疾病的患者。
1.2操作方法
对对照组实施常规院前急救护理,主要包括:接听急诊电话、安排出诊、现场救治、途中护理、入院并检查、等待手术、进行手术。
对观察组实施改进院前急救护理流程,成立专业护理小组,详细分析患者的病情,据此制定科学的改进院前急救护理流程方案:主要包括:(1)电话指导:接到电话后在1min内立刻出诊,争取在15min到达现场,在路上可远程指导患者家属对患者进行初步救治,清理患者口腔和呼吸道,争取救治时间。(2)现场急救:迅速评估患者病情选择抢救方式,保持患者平卧并固定头部,使用脱水剂降血压同时高流量给氧,使用简单呼吸器防止出现呼吸抑制。(3)途中护理:搬运患者时注意将其头部微微抬高,路上避免颠簸,实时监控其心跳、血压、脉搏、呼吸,若患者尚有意识则进行心理疏导。(4)入院前救治准备:路上即联系医院,提前准备手术室和相关治疗器械和工具,安排主治医生和护理团队,开通绿色通道。
1.3观察指标
(1)统计急救效果指标,包括出诊到达时间、急救介入时间、待手术时间以及患者48h内死亡率几个指标,时间越短且死亡率越低则表示急救质量、效率越高。(2)统计并发症发生率,包括肺部感染、脑疝、消化道出血几个指标,并发症发生率=(肺部感染例数+脑疝例数+消化道出血例数)/总例数x100%。
1.4统计学处理
文中计数(χ2检验)、计量(t检验)资料用SPSS20.0软件处理,P<0.05表示数据差异有统计学意义。
2结果分析
2.1急救效果指标
结果:观察组出诊到达时间、急救介入时间、待手术时间均短于对照组且48h内死亡率低于对照组(P<0.05)。见表1。
